What is the Support at Home program?
The Support at Home (SaH) program is an Australian Government aged care program that helps older people live safely and independently at home. From 1 November 2025, it replaced the Home Care Packages (HCP) program and the Short-Term Restorative Care (STRC) Programme.
Support at Home provides tailored services matched to your needs, with different funding levels. It also includes specific pathways for restorative care and end-of-life care, where required.
This program is designed for older Australians whose care needs are higher or more complex than what can be supported through the Commonwealth Home Support Program (CHSP). If you need coordinated support with everyday tasks or more intensive care, Support at Home may be the right option for you.

Who is eligible for Support at Home program?
To access Support at Home, you must:

Register with My Aged Care

Complete an aged care assessment to confirm eligibility and care needs
If approved, you will receive a Notice of Decision and support plan, outlining your funding level and the services you can access.
Most participants will also complete an income and assets assessment, usually through Services Australia, to determine any fee contribution for government-subsidised care.
